Blackbox Healthcare Solutions is a national, clinician-led healthcare operations firm that designs, builds, fixes, and runs statewide and regional transfer coordination centers for state health departments, universities, and health systems. When a rural or tribal hospital needs to move a patient to a higher level of care, it should take one call to find the right bed.
We build the operation that makes that possible: intake and acceptance workflows, a staffing model, governance across health systems, and reporting a state can stand behind. We build on the technology you already have. We don't sell software.
We look at how transfers actually move across your state today: who calls whom, where requests stall, which hospitals accept, and what data already exists. Then we give you a clear, prioritized plan. Typical outputs are a current-state assessment and a 30-60-90 roadmap.
We design and stand up the center: the operating model, staffing model, transfer and escalation workflows, multi-system governance, and a KPI framework. We support you through implementation and go-live, on your existing technology.
Is a coordination center already running but underperforming? We find what isn't working, such as acceptance delays, gaps in capacity visibility, or reporting no one trusts, and tune it until it performs.
We can run the center for a state or university, or alongside your team, until it runs on its own. We operate AZ REACH on behalf of the University of Arizona.
AZ REACH coordinates acute-to-acute transfers for rural and tribal Arizona hospitals. The program is administered by the University of Arizona, funded through the Arizona Department of Health Services (ADHS), and operated by Blackbox. From 12/13/2022 through 9/30/2026 it handled 23,762 transfer requests, 90% were accepted, and the median time to placement was 1.5 hours.

Blackbox's founders and team built and ran the Arizona Surge Line, a statewide COVID-19 transfer and load-balancing center launched for ADHS in 2020. It handled about 12,000 patient referrals from 2020 to 2022, and a peer-reviewed study looked at equity as an outcome of the service.
Indiana RHTP Medical Operations Coordination Center (MOCC): Selected by the Indiana Department of Health in September 2026 to help establish a 24/7 statewide transfer and EMS coordination center under the Indiana Rural Health Transformation Program. Work is underway, and no results have been reported yet.

OHSU News reported on May 1, 2023 that OHSU built the prototype of the Oregon Behavioral Health Coordination Center's (OBCC) real-time capacity visibility tool with partners that included Blackbox Healthcare Solutions. The OBCC tracks inpatient and residential behavioral health capacity across Oregon. Our role was limited to that prototype. The OBCC is operated by OHSU, and its volumes are not Blackbox outcomes.

A statewide transfer coordination center is a single point of contact that helps hospitals, especially rural, tribal, and critical access hospitals, find an accepting facility and bed when a patient needs a higher level of care. It coordinates the request, the acceptance, and the handoff across health systems that would otherwise be called one by one.
No. We build the operation on the technology you already have. Blackbox doesn't sell software. We design the workflows, staffing, governance, and reporting that make the tools work.
Yes. We can run the center for a state or university partner, or alongside your team until it runs on its own. For example, we operate AZ REACH, which is administered by the University of Arizona and funded through ADHS.
We set up a KPI framework at the start. Common measures are transfer requests, acceptance rate, and median time to placement. AZ REACH publishes its figures in a public Reporting Hub.
Each state decides how to use its funds within its CMS-approved RHTP plan. Indiana's statewide MOCC, which Blackbox was selected to help establish, is being developed under Indiana's RHTP.
