Blackbox Healthcare Solutions is a national, clinician-led healthcare operations firm that helps state health departments, rural health offices, and Rural Health Transformation Program (RHTP) leaders turn approved plans into working operations. Our focus is statewide transfer coordination, hospital capacity visibility, and support for rural and critical access hospitals.
RHTP is a $50 billion CMS program, with $10 billion available to states each year from 2026 through 2030. CMS announced awards to all 50 states on December 29, 2025. The funding is in place. The hard part is building operations that last after launch.
We review the transfer, coordination, and capacity initiatives in your plan against how patients move across your state today. We talk with hospitals, EMS, and partners, review the data you have, and give you a prioritized roadmap that lines up with your plan's milestones.
We design and stand up the operation your plan calls for, such as a statewide coordination hub, its staffing model and workflows, governance across hospitals and EMS, and a KPI framework that supports your reporting.
Is an initiative already funded or running? We find what's holding it back and tune it until it performs.
We can run the operation for the state or a university partner, or alongside your team, until it runs on its own.

States set their uses in CMS-approved RHTP plans. According to CMS, the program aims to expand access to care in rural communities, strengthen the rural health workforce, modernize rural facilities and technology, and support new care models. Check your state's approved plan and CMS terms for specifics.
